In-Person QMAP Class Disclosures:

This CDPHE-Approved QMAP course does not constitute any certification or license; offers opportunity for Qualification only as recognized by the Colorado Department of Public Health and Environment QMAP Program. Registration fee includes mandatory $14.00 CDPHE Registry Fee.

Students must be at least 18 years of age and must be proficient in written and spoken English. Signed Disclosure required, provided with study materials


Reschedule Requests & Class Cancellation/Postponement:

Student Reschedule Request -

No refunds offered. Reschedule requests must be received at least 4 calendar days before scheduled class in writing via email ( resulting in credit for an equal QMAP class in the next 60 days. Untimely requests less than four calendar days before class or day of class no-show will result in forfeit of all fees paid. Only 1 reschedule permitted per registration.

Event Cancellation -

In the event of a class cancellation initiated by LifeLearnU for unforeseen circumstances including, but not limited to: community health concerns, severe weather, civil unrest that places students or instructors at risk of physical harm, or Act of God, students may elect one of the following options:

1) Reschedule for another class within the next 60 days. Students may be offered an opportunity to switch to our online QMAP course format.


2) Request a cancellation with partial refund of $80.00 (registration fee minus non-refundable third party processing fees)

Identical class/testing location or week day availability is not guaranteed.

In the interest of community health, our Instructors reserve the right to send students in attendance home for suspected illness - LifeLearnU will offer an opportunity to reschedule free of charge.
