Online QMAP Course Disclosures:
This CDPHE-Approved QMAP course does not constitute any certification or license; offers opportunity for Qualification only as recognized by the Colorado Department of Public Health and Environment QMAP Program. Registration cost includes mandatory $14.00 CDPHE Registry fee.
Students must be at least 18 years of age and must be functionally proficient in written and spoken English and must not have had a medical license revoked for reasons of medication or drugs. Signed Disclosure required day of testing, provided with study materials. U.S. Citizenship is not a requirement for this course. All students must present government-issued photo ID on day of testing.

Required Online Course Completion & Access:
Students receive online access to the QMAP Course for designated dates prior to testing. Specific dates provided upon registration. Individual access code provided on course start date with study materials and worksheet attachment via email.
Completion of online course modules and associated quizzes is required no later than posted online closing date. Students who have not completed all modules are not eligible for in-person testing and will forfeit all fees paid.
Students must bring completed worksheet assignments and disclosure form to in person testing.
Reschedule Requests & Test Cancellation/Postponement:
Due to high demand, students commit to their scheduled test date upon finalizing their registration. All registrations are considered final. No refunds offered, registration is non-transferable. Rescheduling is not available. Non-Completion & No Show forfeit all fees paid without entitlement to refund or rescheduling. Students who are ineligible for testing based on non-completion of the course by the posted due date forfeit all fees paid and must re-register at the standard rate. Class begins promptly at the posted time; students arriving more than 10 minutes late to testing may be denied entry by the instructor & require a paid re-scheduled testing date.
Test Failure:
Passing grades are not guaranteed. Grades are Pass/Fail only: Written exam of at least 85%, practicum success is determined at the sole discretion of our nurse instructor. In the event of failure, a student will have the option to re-register for a discounted fee of $75.00. At LifeLearnU's discretion, a student must repeat the course or engage in a zoom study session with one of our nurse instructors prior to re-taking the written exam. Our nurse instructors will determine if a student must repeat practicum testing on a case-by-case basis. Re-testing practices are in place to ensure a student has gained the necessary knowledge and skills to practice paid QMAP duties safely in a work environment. Registrants must acknowledge & accept these policies prior to final checkout.
Testing Date Cancellation/Postponement:
In the event of a testing day cancellation initiated by LifeLearnU for unforeseen circumstances including, but not limited to: community health concerns, severe weather, civil unrest that physically endangers students or instructors, instructor illness/injury, or Act of God, LifeLearnU will offer an opportunity to reschedule the testing date/location free of charge. No refunds offered. Exact location replacement is not guaranteed.
Illness Policy:
In the interest of community health, our Instructors reserve the right to send students in attendance home for suspected illness.
